Mail-in Ballot Request

To request a ballot, your personal information has to match the information about you that is on file with the Maryland MVA.

These are the things that have to match:

  • Your name
  • Your date of birth
  • Your driver’s license number, ID number, or Learner's Permit number
  • The last 4 digits of your SSN

Your signature at the MVA will become your official voter registration signature.

To use this system to request a ballot, you will need a Maryland driver's license or other MVA issued card, or a Social Security number.

If you have a Maryland driver's license or other MVA issued card, your personal information must match the information on file with the Maryland MVA.
