Voter Registration and Mail-in Ballot Request

A U.S. citizen living in the U.S.

In order to register to vote, your personal information has to match the information about you that is on file with the Maryland MVA.

These are the things that have to match:

  • Your name
  • Your date of birth
  • Your driver’s license number, ID number, or Learner's Permit number
  • The last 4 digits of your SSN

Your signature at the MVA will become your official voter registration signature.

If you do not have a valid Maryland driver’s license or other MVA issued card (including a learner’s permit) or SSN:
- Use this voter registration form to register to vote or update voter registration information.
- Use this mail-in ballot request form to request for a mail-in ballot.

Note: If you are registering for the first time and also requesting a mail-in ballot, you must submit both forms.

To register online, you will need a Maryland driver's license or other MVA issued card, or a Social Security number.

If you have a Maryland driver's license or other MVA issued card, your personal information must match the information on file with the Maryland MVA. The information that must match is your name, your date of birth, and your driver's license or State ID number. Your signature at the MVA will become your official voter registration signature.

If you do not have a valid Maryland driver's license or other MVA issued card, you can provide the last 4 digits of your Social Security number. These four digits of your social security number will be used as your official voter registration signature.

If you do not have the required ID or if you do not want to submit your application online:
- Use this voter registration form to register to vote or update voter registration information.
- Use this mail-in ballot request form to request for a mail-in ballot.

Note: If you are registering for the first time and also requesting a mail-in ballot, you must submit both forms.

Since you do not have a valid Maryland driver's license or other MVA issued card, you must provide the last 4 digits of your Social Security number. These four digits of your social security number will be used as your official voter registration signature.
